PACE of the Triad
PCP (Healthcare)
Performs comprehensive history and physical on new referrals.
Performs interval history and physicals on established participants completed quarterly and as indicated.
Evaluated and treats participants with episodic illnesses.
Refers participants to medical specialists as indicated.
Admits participants to the hospital:
(a) Coordinates primary care with hospital based physicians;
(b) Provides updates to the team on a regular basis;
(c) Judiciously utilizes specialty consultants;
(d) Responsible for coordinating discharge planning with the team.
Manages all care of participants in the nursing home:
(a) Provide regular visit;
(b) Performs telephone contacts with nursing home staff as required;
(c) Admits nursing home participants to hospital when necessary.
Actively participates in family conferences.
Coordination of 24 hour care delivery.
Documents participant changes appropriately in the medical record and communicates participant change to team members.
Participates as a member of the Interdisciplinary Team. Takes part in participant care planning.
Perform all duties related to PCP position per regulatory requirements.
Be accountable to PACE organization.
Cooperate with the competency evaluation program and direct participant care requirements.
Attends staff meetings.
Other duties as assigned.
Environmental Factors
Tasks with potential for exposure to blood borne pathogens include, but are not limited to the following:
Conducting assessments of participants
Performing procedures in the clinic
Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
