Redwood Cove Healthcare Center in Ukiah is looking for a detail-oriented and organized LVN MDS Coordinator to join our team. This is a full-time position with a competitive starting salary of $85,000 per year.

Key Responsibilities:
- Oversee the MDS (Minimum Data Set) assessment process to ensure accuracy and compliance with federal and state regulations
- Coordinate with interdisciplinary teams to develop and update resident care plans
- Conduct assessments and ensure proper documentation to support reimbursement and quality outcomes
- Monitor and manage resident assessment protocols (RAPs) and quality measures
- Collaborate with nursing staff and other departments to ensure residents receive appropriate care
- Stay up to date with changes in MDS regulations and provide staff education as needed
Who We Are Looking For:
- Licensed Vocational Nurse (LVN) in the state of California
- Previous MDS experience preferred but willing to train the right candidate
- Strong organizational and analytical skills
- Ability to work collaboratively with interdisciplinary teams
- Commitment to providing high-quality care for our residents
