Case Manager Job Coach (Creative Design)
Assist in the development of a systematic referral and intake process. Provide case assessment, service plan development, linking/coordination, re-assessments and follow-along/ monitoring activities for assigned consumers.
Document service needs for individual plan development.
Coordinate the annual development of an Individual Plan of Service and its integration between programs, to address consumers identified needs.
Prepare case notes and required annual and quarterly reports to document consumer progress.
Integrate and coordinate services with other human services organizations and within Goodwill Industries to ensure plan implementation and a safe work environment.
Manage consumer records in accordance with organization policies and procedures, and appropriate Medicaid, licensing and accreditation standards.
Provide on-the-job training and/or follow-along: support to employed consumers as required to maximize job retention.
Participate in supported employment presentations to community agencies and referral agents.
Participate in meetings as required.
Keep informed of related professional developments and trends in the field.
May be required to obtain certification in CPR and/or First Aid for benefit of clients health and safety.
Cooperate in coordinating assignments with other staff to provide necessary coverage to serve clients, employees, donors and other customers of the organization.
Maintain quality and integrity of services for consumers.
To perform all duties to the standards established by the organization and all appropriate funding sources or regulatory bodies.
Perform other reasonable assignments as required.
